If you are a physician and would like to refer your patient, please fax your referral in addition to supporting documentation to 905-231-3880. We accept the Palliative Care Common Referral Form. Click here for our referral criteria.
If accepted, your referral will be assigned to one of our team’s palliative care physicians who will contact the patient directly and arrange for an initial consult visit.
We endeavour to see non-urgent referrals within 1-2 weeks. If a patient needs to be seen more urgently, the referring provider must call the on-call palliative care physician at 905-619-1353.
Care is not assumed by the Ajax-Pickering Palliative Care Team until the patient has been seen at the initial visit and has been deemed appropriate by the consulting physician.
