Criteria

Our home-visiting team is only accepting referrals for:

  1. Cancer patients with prognosis < 6 months with PPS ≤ 50% regardless of goals of care
  2. Non-cancer, non-dementia patients with prognosis < 6 months with PPS ≤ 40%, with goals of care aligned with comfort measures (i.e., DNR, no life-prolonging treatment, or plans to return to acute care)
  3. Dementia patients requiring end-of-life care with PPS ≤ 20% (drinking sips or less, or NPO), with goals of care aligned with comfort measures (i.e., DNR, no life-prolonging treatment, or plans to return to acute care)
Palliative Performance Scale (PPS)

We endeavour to see non-urgent referrals within 1-2 weeks. If a patient needs to be seen more urgently, the referring provider must call the on-call palliative care physician at 905-619-1353.
