• Issues with pain pumps, supplies, equipment needs, etc. should be directed to your nursing agency’s 24/7 phone line.
• To contact your physician: please call the on-call number (905-619-1353) and follow the prompts to reach your own physician (Monday-Friday 8am to 5pm) or the on-call physician (after hours, weekends, and holidays).

For prospective patients

• You must first be referred by your physician. Please direct them to our Referral page.

For physicians

• To refer your patient, please see our Referral page. Urgent referrals require a physician-to-physician handover – please call the on-call number (905-619-1353).
